My version is an old one since I switched to Endpoint Manager. But, you upload the logo into the logo's section, then assign it to the phone in the phone's section. I had my logo working there too.

Youn should be aware there is a bug in Asterisk dealing with broken BLFs. It's fixed in 11.21 and 13.7. https://issues.asterisk.org/jira/browse/ASTERISK-25400
