INVITE received but extensions not responding/connecting overtime

Hi, i’ve been trying to setup a small FreePBX system and have mostly got it going after many bumps in the road.

My issue is that, apparently randomly, incoming calls hit the PBX, i get the INVITE listed in sngrp but the extensions(?) don’t connect or respond…
I have two Yealink phones.

A lot of the time the calls are fine and they go thru ok.

I’ve messed around with firewalls / port forwarding /NAT etc and nothing seems to make a difference.
Always seems to be random.

How can i dig down further into this problem?
Any help would be appreciated, feel like i’m going mad!

Here’s one of the INVITE requests:- 192.168.20.10 is the PBX server internal to my network.
